What is Patient Information?

Patient Information is the data gathered, verbal or writing, which is documented to complete the healthcare documentation process. Things that are gathered include:

• Patient Demographics
• facts about the patient
• ex. name, address, phone number, gender, date of birth, ethnicity
• Personal information
• ex: living situaion, diet, activites
• Medical information
• ex: changes in health or activities of daily living (ADL)

Gathering Patient Information begins when the patient first make an appointment.
